The process for a new, first-time patient, starts with the submittal of a Patient Profile, a signed and dated Customer Agreement* and at least one valid prescription. This can be done online, or by filling out our printable forms and faxing or mailing them to us. 

- Ordering Online 

Ordering online is very easy. Go to our New Orders page, and click on the New Patients-Order Online link. 

Read through the steps shown under New Patients-Order Online and click the Proceed to Online Forms link.

Once you have filled out the the online Patient Profile you will need to print out our Prescription Fax Cover Sheet on the "Thank You for Ordering" page. Use this cover sheet to attach your prescriptions, and fax it to us. Please make sure that you do not overlap any of the prescriptions on the fax cover sheet. If you have several prescriptions please print off more fax cover sheets.

* When signing up online, clicking the "I Agree" button on the Customer Agreement is the same as signing the customer agreement. However, if the patient is under the age of 19, they will still need a Customer Agreement signed and dated by a parent or guardian, in care of the patient, which will have to then be faxed or mailed to us.

- Ordering by Mail or Fax

Ordering by mail or fax is very easy. Go to our New Orders page, and click on the New Patients-Fax/Mail Order link. 

Read through the steps shown under New Patients-Fax/Mail and click the Proceed to Printable Forms link.

Once you have filled out the the Printed Patient Profile, and signed the Customer Agreement, you will need to fax, or mail them, along with your original prescriptions to us.

The process for an existing patient varies depending on what type of order is being placed. As an existing patient, the types of orders available are: New Prescription Order, Refill Order and Refill Order with New Prescription.

- New Prescription Order

To order a new prescription as an existing patient, simply print off the fax cover page, attach a copy of your prescription, and then fax or mail it to us.

- Refill Order

Two options exist for ordering a refill of a prescription that we have previously filled for you. They can be ordered online, or by phone.

To order online, simply go to our Refill Orders page and click on the link titled: "Online Refill Order Form", then complete and submit the form.

To order refills by phone, simply call our toll-free customer service number, and request to order a refill of your medications.

- Refill Order with a New Prescription

Just like regular refill orders, two options also exist for ordering a refill with a new prescription They too can be ordered online, or by phone. As well, we will also need a copy of the new prescription.

To order online, simply go to our Refill Orders page and click on the link titled: "Online Refill with New RX Order Form", and complete and submit the form. After the form is submitted we will also need you to send in your new prescription. To do this, simply print off a fax cover page, attach a copy of your prescription, and then fax or mail it to us.

To order refills by phone, simply call our toll-free customer service number, and request to order a refill of your medications and ask for it to be shipped with your new prescription. Then our  customer service representatives will ask for information regarding your new prescription to help us match it to your refill.
